Completing a Self-Assessment is the first step in preparing for your accreditation.

Tips for completing the self-assessment:

  • If the yellow bar appears at the top of the spreadsheet saying “Protected View” then click on the “Enable Editing” button before starting to ensure your input is able to be saved.
  • Save the self-assessment to your hard drive before completing it.
  • The self-assessment attached is made up of four spreadsheets.

Overview – This provides a summary of the Self-Assessment template and has three fields for you to complete.

Practice Description – This only needs to be completed if you have more than one site to be included in your accreditation.

Evidence – This is the main work sheet where you review each indicator and using the drop down options you specify if the indicator has been met. Full instructions for completing the self-assessment are at the top of this Evidence page.

Self-Assessed Improvements – This is automatically populated with any improvement actions entered on the ‘evidence’ tab – you can use this to plan your improvement actions and list staff responsible. Once you have completed the required actions you should go back to the Evidence; worksheet and update the indicator to ‘Met’.

Once you have completed your self-assessment (ie all indicators are ‘Met’ and the declaration has been completed) and are prepared for your assessment, please upload the self-assessment, along with the supporting documentation/evidence records to your onTrack for Dental portal.
